Two formulas cover most requests. Total recordable incident rate equals recordable cases multiplied by 200,000, divided by total hours worked. Lost time injury frequency rate commonly equals lost time injuries multiplied by 1,000,000, divided by hours worked. Clients specify which base they want, so read the form. A wrong formula produces a wrong number, and a wrong number damages trust faster than a bad number.
Personnel Competence: Where Qualifications Matter Most

Large clients know that most site incidents involve someone lower in the contract chain. So they read your subcontractor management with a suspicious eye. Show a chain of control with five links.
